Make an Appointment
You can make an appointment by calling our office at 219-462-6144, ext. 309 or 877-462-6249 during normal phone hours, 9:00 am to 4:30 pm, Monday through Friday.
You can also request an appointment by filling out the form. Please tell us the nature of your visit and possible dates that would work for you. You will receive a return e-mail within two business days to verify your appointment or to suggest an alternative.
Our No-Show or Late Cancellation Policy
OGA will charge a fee of $75 for all missed appointments (no-shows) and for appointments rescheduled within 24 hours of the appointment time. No-showing or rescheduling within 24 hours does not give OGA enough time to allot the time to another patient who may be waiting for care. The $75 fee will be charged to the patient.
